Thousands of medically fit patients will be discharged safely from hospitals into community care homes over the coming weeks to reduce pressure on the NHS.
The £200 million will be used to buy care placements allowing more people to be discharged from hospital. The £50 million will be used as capital funding to improve and expand hospital discharge and ambulance facilities.
